Pop & Jazz Recital Entry Form

    Each teacher may submit up to 6 students in each recital. Please check the Rules and Procedures for guidelines on repertoire.
    Enrollment may be limited depending on the total length of the recital program.

    Only online entries will be accepted.  
    Fee is $10 per entry(solos or ensembles) for members, and $15 for non-NWSMTA members. 

    For Fall Pop & Jazz Recital, mail payment to: Chyi-Ling Evans, 1068 W Willow Street, Palatine, IL 60067
    For Spring Pop & Jazz Recital, mail payment to: Chyi-Ling Evans, 1068 W Willow Street, Palatine, IL 60067 

    This is how it will appear on program and certificate.
    Students must be at least 7 years old on the day of the recital and have studied at least 6 months with the teacher entering them.
    TIME LIMITS: Elementary: 2 minutes Intermediate to Early Advanced: 4 minutes Advanced: 6 minutes * A teacher's TOTAL TIME of all entrants may not exceed 16 minutes
    If original composition then write none.
    Award Competitions pieces are not permitted in the Spring Recital.
